Coordinating home social-healthcare assistance is not a trivial web-directory problem. When a patient needs care, the time expectation is stringent. But the real technological challenge hides "under the hood": managing a large team of self-employed healthcare professionals in a transparent and scalable way.

## The problem we found

The Gordian knot to untie was twofold. On one side: each specialist operates under a different tax regime (flat-rate VAT, stamp duties, collaboration, etc.), imposing invoicing logic, credit notes and split payments accurate to the cent. On the other side: guaranteeing timely delivery of consultations, both physical and video, integrating different disciplines without operational bottlenecks.

### How did you handle the taxation of Italian professionals?

We designed a custom backend engine on Node.js, integrated with Stripe and Fatture in Cloud. During onboarding, the system profiles each specialist's regulatory regime; at every transaction, it dynamically orchestrates financial flows and document production, eliminating any manual reconciliation work.

### What does it mean to introduce a video-consultation system?

It means going well beyond secure, encrypted streaming. We added a real-time chat layer and transactional emails alongside the video call. Information and prescriptions exchanged during the session converge into automatic reminders sent at the end of the consultation, centralizing the entire booking cycle (virtual or in person) in a single platform and eliminating the need for external tools (Google Meet, Teams, WhatsApp and the like).

### How do professionals interact with patients and administration?

Through a dedicated App developed in React Native. This centralizes clinical operations (emergency alerts, visit acceptance) and the bureaucratic cycle: downloading invoices transacted via Stripe, monitoring fees and contacting the support desk. We automated the infrastructure to make the delivery and accounting-management flow as natural and simple as possible.
